IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
Notice of Availability of Form 1095-C
This notice is to inform current and former employees of the Suffern Central School District that Form 1095- C, Employer-Provided Health Insurance Offer and Coverage, is available upon request for tax year 2025 and forward.
Under new requirements set forth by the Paperwork Reduction Act, employers are no longer required to automatically mail paper copies of Form 1095-C. Instead, we provide this notice to inform you that the form is available if you need it.
Form 1095-Cs provide information about offers of coverage made to full-time employees and enrollees in self-insured coverage.
Please note: You do not have to wait for either Form 1095-C from your coverage provider or employer to file your individual income tax return. You can use other forms of documentation, in lieu of the Form 1095 information returns to prepare your tax return. Other forms of documentation that would provide proof of your insurance coverage include:
- insurance cards,
- explanation of benefits
- statements from your insurer,
- W-2 or payroll statements reflecting health insurance deductions,
- records of advance payments of the premium tax credit and
- other statements indicating that you, or a member of your family, had health care coverage.
You will not need to send the IRS a copy of Form 1095-C or proof of your health coverage.

Requests can be made at any time, and the form will be provided to you by March 2, or within 30 days of your request, whichever is later.
